In October 2019 Illy Systems Ltd launched a brand new website that will be available to all Bridge members.
WHAT IS myCAREPATH?
myCarepath is a mobile and tablet friendly website used by members to:
You will be able to view all your upcoming appointments and the location of the appointment.
If a post code has been included with the location address, then you will be able to view a map.
VIEW USEFUL DOCUMENTS
You will be able to download any document that your worker has shared with you. This includes any generic information that you feel may benefit you e.g. fact sheets, and also any documents that you would like copies of e.g. Housing applications.
Your worker can share useful contacts with you – these may be Bridge workers or details of other professionals.
You’ll be able to message your worker via myConversations.
Your worker will be alerted to any unread messages once they log-in to CarePath.
All messages are logged within your LINKS CarePath record.
DAILY USE DIARY
You’ll be able to keep a log of your daily substance use via myCarepath.
This will be automatically saved onto your Carepath record for Bridge staff to see.
You can change your password, contact details and username at any time.
Two-factor security settings are easily set-up once your myCarePath account has been activated.
You can request for your myCarePath account to be closed from your Profile page
Once you have started to use myCarepath, any information that you have logged will be visible to all Bridge staff that have Carepath access and S2S staff if, and only if, you have given Bridge consent to share information with S2S.
